Geriatric
care managers work privately with older adults and their families to
create a plan of care that meets the needs of the older adult. They will
meet with you to help you understand your loved one's needs and learn
what resources and options are available to meet those needs.
If
you are asking yourself the following questions, a geriatric care
manager is the answer:
- Are my parents safe at home?
- Are bills being paid on time?
- Are my parents' health concerns taking me
away from my family?
- What kind of help would increase safety
while maintaining my parents' dignity and independence?
What does a geriatric care manager do?
- Determine client needs and help locate
services.
- Provide comprehensive health assessment
completed by an R.N.
- Work with community resources for social
support.
- Facilitate family meetings to help with
care decisions.
- Work with client physicians to ensure
effectiveness of prescribed treatments.
- Provide communication and support for
families who live at a distance from elder relatives.
- Assist with aging issues, crisis
intervention, medication management, home safety, rehabilitation
plans, pain management, and wound care.
- Compiles an assessment of an older person's
needs and situation.
- Encourages the person to accept help and
provides a "plan of care" with specific recommendations.
- Finds and secures services such as legal
counsel, home care, nursing care, or home maintenance.
- Supports and counsels family members.
